This page will be maintained to keep our EFORMS users informed on any changes made to the forms posted on this site. We will list each form by ANSC form number (or "Approved Form Number"), title, and revisions to form files, as they are posted.
This listing will include all form changes, including base form (blank) changes and form field changes. If the form change obsoletes a previous form date, which can occur only when the actual form date is changed, a notice will be posted both here, and on the National Website's "What's New" page. If the revision does not affect the form date, or if the previous form may still be used, the announcement will be included here, but not necessarily on the "What's New" page.
NOTE: To see the form's "Revision Number", which refers to the Acrobat PDF file revision, open the form using Reader. Press Ctrl-D to display the document information, and view the Title: box for the Rev-xx entry. The last modification date of any form is listed as the Modified: date.

The Questionnaire for National Security Positions, SF-86 e-Form has been revised. Question 21, which addresses medical records, has revised language regarding mental health counseling. The form itself is not yet revised, but a popup Help box is now included with the new language. These new instructions were effective 1 May 2008 and apply to all using this form.

The Vessel Safety Check (VSC) form, ANSC-7012 is revised. The Boating Safety Hotline phone number has been discontinued and has been deleted from page 2 of the form. The previous edition may be used until the supply is exhausted, with the requirement that the number be obliterated when used.

The Activity Report-Mission, ANSC-7030 (3-08) is revised. Added is mission 22D - Sector Coordinator mission, and changes to mission descriptions for 06A (deleted Operations/Air workshops) and 22A (replaced Safety Patrol with MOM). Also corrected are the callouts for 70M and 70N in the mission drop-downs. The previous edition may be used until the supply is exhausted.

Three Coast Guard Auxiliary Association, Inc. forms, CGAuxAI-1, CGAuxAI-3 and CGAuxAI-4, all dated 01-08, are revised to include a field for BUDGET ACCOUNT. The previous editions are obsolete.

The Activity Report-Mission, ANSC-7030 (1-08) is revised to correct four errors on page three. The Health Care mission description changes were inadvertently missed, and Mission 70D erroneously included "R" for radio as an applicable resource. There are no other changes.

In addition to the changes listed on 1-02-08,the Activity Report - Vessel Examinations, ANSC-7038 (1-08) now allows multiple inspectors for M missions. The same method as used on the 7030 for trainees is incorporated here. The change does not affect VSCs or Facility Inspections; copies of the previous edition may be used until supplies are exhausted for those missions.
The Unit Officer Report, ANSC-7007 (1-08) is revised to reflect current authorized offices for flotillas and divisions. The previous edition is obsolete.

Added to the required attachments to the Membership Application, ANSC-7001 (1-08) is a copy of the document used to verify citizenship. The previous edition is obsolete.
The Activity Report-Mission, ANSC-7030 (1-08) instructions have been extensively revised to reflect the mission consolidation effort. Since only the picklists for the missions have been revised in the Acrobat fill-in form for page 1 and page 1's only change is the form date, the previous edition paper copies can be used until the supply is exhausted. The changes are found in pages 2 through 8. Missions whose names or definitions have changed are denoted by underlining their names on the mission description pages.
ANSC-7005 - Aircraft Offer For Use (1-08) is revised to include the requirement that Time Between Overhaul (TBO) be checked for applicable aircraft components. The previous edition is obsolete and may not be used.
The Activity Report - Vessel Examinations, ANSC-7038 (1-08) has had its AUXDATA callouts revised to reflect the new mission activity and submission choices in AUXDATA. There is no other change to the face of the form; copies of the previous edition may be used until supplies are exhausted.

The Membership Application, ANSC-7001 (10-07), has been reorganized to eliminate the requirement that multiple copies be submitted to DIRAUX/SECCEN. Also added is information required for the ID card, as well as reinforcement that submitting to the PSI process does not necessarily guarantee acceptance as a member. Enclosures (1), (2), & (3) to COMDTINST M16794.51A are now available for downloading on the Auxiliary Boat Crew Training Manual Appendices & Enclosures page. These are used in conjunction with the triennial Currency Maintenance/Recertification Check Rides for Crewmember, Coxswain and PWC (respectively).

The five new appendices from the Auxiliary Boat Crew Training Manual, COMDTINST M16794.51A, are now posted on the Forms site. They are located on their own page: Auxiliary Boat Crew Training Manual Appendices. All will be converted to fill-ins in the near future.

All four Facility Inspection forms, Vessel (7003), Radio (7004), Aircraft (7005) and PWC (7008) have been revised to include facility availability questions. The four previous editions are obsolete and may not be used. Added to the facility group is a new Vehicle Offer for Use form, ANSC-7065.
The ATON Verification form, ANSC-7054, has been updated. The previous edition may not be used.
The Membership Application, ANSC-7001, has been substantially updated to include all previously-attached forms as a single form. The only attachments required are two fingerprint cards, and if a previous clearance was granted, supporting documentation of that clearance. Please note that DIRAUX will require original copies of pages 1, 2, and 3, and in addition, SECCEN will require original copies of pages 1, 2, 3, 4, & 5. All signatures on those copies must be originals in ink. The previous edition may not be used.
All of these forms are dated 1-07.

NEW (sorta!) FORMS

The three forms required for Operational Support-level Personnel Security Investigations (PSI) have been combined in to a single file. OFI-86C, SECCEN Verification of Citizenship form, and page 6 of the SF-85 are designed so that as you fill out the first form, pertinent info trickles immediately into appropriate boxes in the other two forms, so that duplicate entries don't have to be made.
DHS-10000-9 (10-04) is now available as a fill-in and is also now posted on the Security Forms page.

The National Staff Appointment Request, ANSC-7061 has been revised to include the staff position the appointee reports to. The new form date is 10-05. The Direct Deposit Request Form for Mail/FAX use - FMS-2231-CGAUX (07-05), replaces the FMS-2231 previously posted. This form includes all information required by FINCEN to initiate electronic deposit of patrol reimbursements and travel expenses directly to the member's bank account. Use this form if the member does not have internet access and must mail or FAX the information to FINCEN. FINCEN prefers members use the on-line method if possible.

Three facility forms, Vessel Facility Inspection and Offer for Use, ANSC-7003, Aircraft Facility Inspection and Offer for Use, ANSC-7005, and PWC Facility Inspection and Offer for Use, ANSC-7008 are revised effective 01 January 2005. The previous editions are obsolete and may not be used. These forms no longer carry a CG form number, and all include LAT-LONG fields, along with other substantial changes. The Annual Unit Officers Report, ANSC-7007 (11-03) now has eSignature capability, and the DIRAUX signature has been eliminated. Normal distribution includes an information copy for DIRAUX, but signature is not required. The previous edition may be used until supplies are exhausted. The Change of Officer Report, ANSC-7006 (11-03) has been revised, also eliminating the DIRAUX signature. eSignature is also enabled, and the unit number is now included in the upper right corner. The previous edition is obsolete. The Temporary Duty Request and Travel Order, CGAuxAi-1 (1-02), is now included on the regular Forms page. CGAuxAi-2 has been rendered obsolete with the use of CGAuxAi-1.

The Activity Report- Mission, ANSC-7030 (04-03), reflects changes to the descriptions for 28L and 28P. Instructions have been revised slightly for missions 09, 23, 70K, and 90C. The previous edition (2-03) may be used until supplies are exhausted. The Activity Report - Vessel Examinations, ANSC-7038 (04-03) clarifies text to reflect that only Vessel Facility Examinations can reported on this form- Air and Radio facilities cannot. The previous edition (01-03) may be used until supplies are exhausted.

The Examination Answer Sheet, ANSC-7010 (2-03) - CG-4886, is now reformatted for printing on users systems. Notice that the pattern has been rotated 90 degrees so that printers that crop edges will not miss part of the pattern. The pattern itself matches the 11-76 edition- only the orientation is changed. Also, note the 8" line-- users must make sure their printer images this line at 8" long, or the pattern will not match existing answer keys. At the present time, there is no plan to offer this form as fill-in. The previous edition may be used.

The Activity Report- Mission, ANSC-7030 (2-03) has been updated to include PE course completion information. Users of this edition will no longer need to submit ANSC-7033 to report PE statistics. Members reporting PE activity other than the last mission for a class must not enter data in the course completion section, with only one member making the entry for each completed class. The previous edition (1-03) may be used until supplies are exhausted; form 7033 would have to be used also to report class completion with that edition. When supplies of the 1-03 edition are exhausted, form 7033 will become obsolete. The AIM Application, ANSC-7049 (12-02) now clarifies for applicants that the FSO-CC and DSO-CC information will be completed by the AIM officer. Only two lines in the instructions were affected- 12-02 dated forms may still be used, but CC officers are encouraged to make sure any forms distributed for applicants to complete themselves include the instruction page from the form posted today. The Acrobat (ctrl-d) revision will show 002, and the Acrobat modified date will show 01-31-03.

The Activity Report- Mission, ANSC-7030 (01-03) will now display mission number choices in multiple panels. A problem exists with Windows 95 and Windows NT machines which does not allow the choices to scroll when more entries are available than can be displayed on the screen. Also, a correction was made to the description for mission 10H which now reads: "DIRECT MAILINGS: The time spent preparing and mailing information/promotions to the public." The description previously was in error.

The Workshop Mission and Attendance Report, ANSC-7039 (10-02) and The Member Activity Log - ANSC-7029 (10-02) have had minor changes made in their instructions. The workshop form instructions now reflect the possibility that the sponsoring unit may not be the same as the instructor's unit. The WORKSHOP TYPE instructions have had "national" removed, as all workshops are not necessarily "national". The Activity Log has expanded guidance on its use instead of the 7030, and a suggestion for A.2 to see a following paragraph for examples. Neither form's revision date has been changed; previous forms (dated 10-02) may be used until supply is exhausted.

The Claim for Reimbursement-Travel, CGAuxA-3 (8-01), and Claim for Reimbursement - Non-Travel, CGAuxA-4, (8-01) have been revised. The previous editions are obsolete. Both now have spaces for the submitter to enter a phone number, and CGAuxA-3 has added boxes to check whether previous payments have been received. CGAuxA-3 also has had the text regarding the time allowed to submit claims changed to 7 days after travel is completed, instead of 7 days after incurring the expense.

Workshop Attendance Report. ANSC-7039 (6-01) as had the column for "Trainee" removed. IS personnel are instructed to enter "N" for everyone listed on the report. Item 15, previously a write-in line, has had Team Coordination Training assigned to it. The previous edition may be used until supplies are exhausted. Note: a new feature, automatic date entry, is included on this form. This feature will work with Reader versions 4 and above. Also, a fill-in field has been added for "member signature" in anticipation of electronic delivery of form data. Consult with your IS officer as to whether your actual inked signature is required on this and subsequent form updates.
